SPAY NEUTER SERVICES OF INDIANA, INC. Assistance Application
If you are part of a Needs Based Assistance Program in Indiana (Food Stamps, Medicaid, SSD, SSI, WIC, Section 8 Housing) you may be eligible for a reduced spay/neuter fee of just $20 through the Spay Neuter Assistance Program (SNAP) of Spay-Neuter Services of Indiana (SNSI). Please download the application below and carefully read through all the requirements. Mail your application, payment (money order), and required proof of limited income to SNSI. You will be notified within two weeks in regards to whether or not your application was accepted and they will provide you with a voucher that you can use at the H.O.P.E. for Animals Spay/Neuter Clinic. Once you get the voucher, call and schedule an appointment and let them know you'll be using a SNAP SNSI Voucher.
The application needs to be mailed to Spay/Neuter Services of Indiana NOT HOPE for Animals
